Each student must submit a completed enrollment packet which includes the following forms:
- Student Record Checklist
- Enrollment Application and Agreement Form
- Confidential Information Form
- Student Medical Form / Food Allergy Emergency Plan Form if applicable
- Updated Immunization Records (signed by the doctor)
- Registration, Tuition and Refund Policies
- Tuition Express Debit Card Form
- Land’s End Instructions
- Children’s Get to Know Me (Children Enrolled in 18 months and 2’s Only)
These forms must be filled out truthfully and completely. An annual non-refundable registration fee, supply fee, and the last month’s tuition are due upon registration to secure your child’s place in the program.
Once completely filled out, print and bring them into our office.
